The following steps outline the subscription process:
- Determine the type of subscription you want
- Enter your registration information for your business. Please note that your information is kept in strict confidence.
- Pay your Setup fee of $50.00
- Pay your Setup fee and Appointment Service Package Suite ($245.00) if this is your choice
- Pay the first installment of your subscription - either monthly, biannual or annual payment. This payment is made separately for accounting purposes and for your convenience.
- Complete the Questionnaire to setup your Online Appointment web site
- Complete the form to provide information about your Operators and Locations
All payments are made through a secure payment system provided by Paypal.
